Let me re iterate what Fan said. Both pitchers have to take the mound and throw at least 1 pitch in which the catcher will field the throw. Pitchers must both have 1 foot on the mound. Underhanded pitches count as well.

How about if the first pitcher comes up to the mound, goes through his motions and throws the pitch. A bird flies in the path of the ball. The ball hits and kills the bird ...

Now, the pitchers is a Tree Hugger and President of PETA .... He realizes that the bats are made from trees, his glove is made from cows, and that he was responsible for killing a bird.

He is now an emotional wreck and can not continue the game. WTF happens now KB?
